Our Home Care Services provide the help you need to recover from illness or surgery in the comfort of your own home and live independently with supportive care. 

Long Beach Medical Center’s Home Health Care Agency provides the help you need to recover from illness, injury or surgery in the comfort of your own home. 

As veterans in the industry, we have established a reputation of high quality care, professionalism, and compassion for our patients.  We offer an integrated, multi-disciplinary system of home care services that are specifically designed to meet the health care needs of each individual. 

Through our services we can:

 

• Help patients recover from surgeries and debilitating illnesses.

• Teach patients and families how to manage the patient’s illness/rehabilitation.

• Assist patients and families in accessing community services.

• Provide caregiver support and guidance

• Coordinate private insurance, Medicare & Medicaid benefits.

• Provide short and long term care.

• Serve as patient advocate and family liaison.


We offer two unique programs for home care services:

 

• Our Certified Home Health Care Agency (CHHA) for short term, recuperative care.


• Our Long Term Home Health Care Program (LTHHC) for patients who require assistance on a more permanent basis.


Our Certified Home Health Care Program for Short-term Care:

Following a major illness, injury or planned surgery, your doctor may feel you would benefit from additional health services as you recuperate at home.  Our Home Care Agency can provide all the assistance you need.   

Our personalized programs are coordinated by a Registered Nurse who is licensed by New York State and specially trained in home care. 

Your nurse will coordinate a team of nurses, therapists, social workers and home health aides who will work with you as you recover from surgery, heart attack, stroke, fractures, or debilitating illness.  

Our professional staff is knowledgeable in the management of chronic diseases such as Congestive Heart Failure, COPD, Diabetes, Stroke, and psychiatric diagnoses.  We provide expert Wound Care, as well as a comprehensive rehabilitation program. 

Who is Eligible?

You don’t have to be hospitalized to qualify for Home Care.  A doctor’s referral will initiate an evaluation for services based on your need for intermittent skilled nursing or rehabilitative care. 

Insurance Coverage

Our Home Care department coordinates private insurance, Medicare, Medicaid, and other insurance.  Private payment and a sliding scale of fees can also be arranged.  Our staff will work with each patient and family. 



Our Long-term Home Health Care Program:  An Alternative to Nursing Home Placement 

Through our Long-Term Home Health Care Program, individuals who would otherwise need placement in a skilled nursing or health related facility can continue living at home with dignity and independence. 

Commonly called “Nursing Home Without Walls,” the Long-Term Home Health Care Program’s professional team offers support and guidance and an array of services aimed at maintaining individuals in their own homes. 

Services include:  Nursing, Physical, Occupational and Speech therapies, Nutritional Counseling, and Medical Social Work.  In addition, by contractual agreement we can provide Respiratory Therapy, Audiology and Personal Care Workers.  We can also arrange special services such as:  Emergency Response Systems, Social Day Care, Home Maintenance and Improvement, Respite Care, and Moving Assistance. 

Who Is Eligible?

Nassau County residents who might otherwise require nursing home or related facilities placement may apply for Long-Term Home Health Care, with approval from the NYS Department of Social Services and in conjunction with a physician-ordered plan of care.  Support from family or friends is also required.

Cost of Services

The Long Term Home Health Care Program is covered by Medicaid.
